Age
Limit: Candidate age
must be below 37 years for Staff Nurse post, below 27 years for Cook/ Cook mate
post, between 18 to 27 years for Jr. MRT, Jr. Radiographer, Dresser, Other
Class-IV posts and 32 years for all remaining posts. Cut off date for age
determining would be 31-12-2012. (Age relaxations will be extended as per
rules).
Educational
Qualification: Candidate
must possess Matriculation/ 10+2/ Diploma/ Graduation/ Master Degree/ B. Sc/
Post Graduate Diploma in relevant discipline from a recognized University.
Application
Fee Details: Applicants
need to pay Rs. 125/- for Grade pay 1800 to 2000 posts and 225/- for Grade pay
2001 and above. Candidates are advised to remit the requisite fee in Punjab
National Bank at any Branch. For remittance of fee, candidates have to generate
the prescribed challan from ESIC website. No Fee is required to be paid by
Female / SC / ST / Ex. SM/PWD/ESIC employee/ Govt. Employee candidates.
Selection
Procedure: Candidates will
be selected on the basis of Written Test and/ Interview.
How to
Apply: Eligible
candidates may apply through Online from 07-12-2012 to 31-12-2012. After
submission of application candidate need to take print out online
application form and send along with relevant documents and challan
received from the bank by super scribing “Application for the post of ______ ,
Post Code No. ______ State Punjab’ on the envelope to the following address “to
The Medical Superintendent, ESIC Model Hospital, Bharat Nagar, Ludhiana-141001
(Punjab)” on or before 05-01-2013 by Speed Post or Registered Post.
Instructions
for Online Application:
1. Candidates should first scan their photograph and signature as detailed under guidelines for scanning the photograph, signature & thumb impression
3. Fill the application carefully and submit the application.
4. After submission of application, Candidates should note down the registration number and password for further reference.
5. Candidates need to send the hard copy of application and along with challan received from the bank relevant documents to the above mentioned address on or before 05-01-2013 by speed post or registered post.
