II. At District Level:

1. Epidemiologist – 29 posts

2. Data Entry Operator – 27 posts

3. Data Manager – 15 posts

4. Micro Biologist (District Level) – 01 post

Age Limit:

For State Level Posts: Candidates should have 45 years or below Epidemiologist Posts, Entomologist,

60 years or below for Consultant Post, below 40 years for Data Manager Posts,  Data Entry Manager Post.

For District Level Posts: Candidates should have 45 years or below for Microbiologist Post, 40 years or below for Data Manager & Data Entry Operator Posts.

Educational Qualifications: Candidate must possess Medical Graduation with PG Degree/Diploma in Preventive and Social Medicine/Public Health or Epidemiology for Epidemiologist at State HQS, M.Sc (Zoology) with entomology as one of the subjects for Entomology post, Post Graduate qualification in Medical Field preferably in Public Health/Epidemiology with minimum 5 years experience for Training Consultant post, Post Graduate Qualification in Computer Science or BE in IT/Electronics for Data Manager Post, Graduate with at least one year diploma in Computer Application for Data Entry Operator post. For remaining posts candidates are requested to refer the notification.

How to Apply: Candidates are requested to download the application format from the notification or from the site www.mp.gov.in/health and fill it with all mandatory details, affix photo, attested copies of all educational proofs, age, experience certificates send in a sealed cover super scribe with name of the post to be applied for to the Director, Directorate of Health Services, Fourth Floor, Saathpuda  Bhawan, Bhopal (Madhya Pradesh) for state level posts on or before 25-12-2012 by Registered Post/By Hand.
